Why grinding questions stops working
Many candidates plateau after a few thousand questions and conclude they need a different question bank. Usually they need a different method. Answering a question exercises recognition for a few seconds; reviewing it properly builds the reasoning that transfers to the unfamiliar stems you will meet on the actual exam.
The plateau has a signature. Your percentage hovers in the same band regardless of volume, you recognise questions you have seen before and answer them correctly for the wrong reason, and you feel busy without feeling more confident. When that pattern appears, cut your question volume in half and double your review depth for two weeks.
The comparison worth internalising is this: fifty questions reviewed thoroughly beats two hundred questions skimmed. Thorough review means understanding why the right answer is right, why each wrong answer is wrong, and what the item was actually testing beneath the surface content.

The four-step question routine
Step one is to read the stem and identify what is actually being asked before you look at the options. Rephrase it in your own words: is this asking for the first action, the priority client, an expected finding, or evidence of understanding? Misidentifying the question demand causes a large share of errors made by candidates who knew the content.
Step two is prediction. Before reading the options, decide what you think the answer should be. Prediction protects you from distractor pull, because well-written distractors are designed to be attractive when read cold. If your prediction appears among the options, your confidence is genuinely earned.
Step three is elimination with a stated reason. Remove options you can rule out and articulate why: outside nursing scope, not the priority, unsafe, addresses the wrong problem. Step four is review, which is the step that actually builds ability. Read every rationale, including on items you answered correctly, and write one sentence capturing the transferable principle rather than the specific fact.

Error logging that actually helps
Most candidates log errors by topic, which produces a list saying 'cardiac, endocrine, cardiac, paediatrics' and tells you almost nothing. Log by cause instead. The five causes in the table above account for nearly every wrong answer, and each requires a different fix.
The distribution is diagnostic. If most errors are knowledge gaps, you need content work. If most are misread stems, you need pacing and reading discipline, not more content. If most are strategy errors, you need prioritisation and delegation drills. Candidates who study according to their error distribution improve much faster than those who study according to their topic list.
Keep the log short and usable. One line per error: the cause, the principle you missed, and the date. Review the log at the start of each session for two minutes. That habit alone converts your mistakes into a personalised curriculum.

Spacing, mixing and simulating
Two evidence-backed principles multiply the value of every question you do. Spacing means revisiting a weakness after a delay of a few days rather than immediately, because immediate repetition feels productive but produces short-lived retention. Mixing means practising several content areas within one session rather than blocking a single topic, because mixed practice forces you to identify what kind of problem you are facing, which is exactly what the exam demands.
Add full-length timed simulations weekly in the final month. These do three things content practice cannot: they build stamina for a long adaptive exam, they expose pacing problems while there is still time to fix them, and they reduce the novelty that fuels test-day anxiety.
